Registration for New Students starting School Year 2008

New Georgia Immunization requirements for new entrants to Georgia schools including change in vaccine requirements for all students entering 6^th grade:

Mumps 2^nd dose required or proof of serologic immunity
Varicella 2^nd dose required or health care provider documentation of disease history or proof of serologic immunity.

GWINNETT COUNTY REQUIREMENTS FOR REGISTRATION

You must have all the requirements listed below at the time of registration;

ü Proof of Residency which should be one of the following;

A. Purchase agreement/non contingent for home closing within 90 days

(settlement statement & utility bill due on closing date)

B. Settlement Statement of new purchase along with utility bill

(phone bill not accepted)

C. Rental Agreement along with utility bill

(phone & cable bill not accepted )

ü Withdrawal form from your previous school which should include:

1. most recent report card

2. discipline record or statement stating “no discipline”

3. address, phone & fax number of previous school

ü Immunization record on Georgia Form No. 3231

ü Hearing/Vision/Dental Certificate Georgia Form No. 330

*Additional information will be required for Special Education or Gifted placement
